Day Support Services

We support a wide range of both Centre and community based activities, many of our service users have their service provided in their local area rather than in the Centre itself.Our support emphasises the inclusion of people in their own communities aiming to enable people to overcome social isolation and home pressures. An increase of confidence and ability aims to enable participation in and access to opportunities for education, health and fitness, leisure, employment, cultural and other community activities.

 

For people with Physical Disabilities:

In certain situations the nature and circumstances of disability lead to a significant loss of independence, quality of life, deterioration of home circumstances and an inability to meet the challenges of everyday life.

All people using our day Support Services have an individual personal plan which has agreed goals which people work towards. This enables people to recognise and realise their potential. Service users participate in reviewing their plans, resetting goals and timescales.

 

Support can take a number of forms eg-

Community Groups:-

 

We operate a number of local community groups. These usually take the form of a locally based group and activities can include rehabilitative, educational or socially focused programmes.

These groups are usually based in local community centres or other local facility. In some cases once people have gained the necessary skills and confidence people can be referred on to the "local area co-ordination team" who can help them develop further community interests.

Centre Based:-
For some of our clients with higher needs we will continue to offer a centre based service. In time this is likely to be for clients who need to access specialist equipment such as hoists, specialist seating or special sensory based activities. However these clients will also tend to have many of their activities in a community setting.
Personal Development:-
We aim to improve reduced confidence and low self esteem, thereby increasing the ability of people to overcome challenges and to engage with the mainstream community. This may lead to support and referral to access other services. Clients will have an allocated key worker who will work with them to develop a "personal plan". In this plan a number of personal development targets can be set. These are then regularly reviewed.

 

For people with Learning Disabilities:

Much of the above also applies to our services for people with learning disabilities.
We have a variety of local groups which operate from community bases for which clients take part in a wide range of community based options. For example through these groups clients can take part in adult education classes and attend college as well as many other options.  

For clients with higher needs we offer a centre based option which allows us to provide a range of specialised activities and provide specialised equipment, however many activities will still take place in a community setting. We also have groups aimed specifically at younger and older groups.
