Rehabilitation - Condition Focused Days
For example:
The Firrhill Stroke Day
The aim is to continue rehabilitation in a community setting for people that have had a stroke, following discharge from hospital. Usually the hospital based service is unlikely to continue for long beyond the acute phase of the stroke.
Often people are discharged when able to manage basic living skills, however in some respects rehabilitation in your daily life only really begins at that point.
Participation in the programme also provides an opportunity to gain support from others who are experiencing similar difficulties.
Following an initial period of assessment the service users set themselves a variety of rehabilitative goals. Our staff then work on these goals along with the service user with a view to achieving short term aims.
Each person has an individually planned programme designed to meet their own particular needs.
Much of the rehabilitation programme takes place within our purpose built centre, however we also aim to increase the opportunities for users of the service to develop interests in community settings beyond the centre. For example you may be supported to access a local gym or fitness club.
